What is PACE?  

  • Program of All-inclusive Care for the Elderly
  • PACE is an innovate model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live an independently as possible.
  • PACE programs coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community.
  • PACE utilizes an interdisciplinary teams – including physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – to exchange information and solve problems as the conditions and needs of each individual who decides to participate in PACE change – all with the objective of enabling participants to live longer in the community.

Physical Therapist

Traverse City, MI

PACE North is seeking a Physical Therapist for its new Program of All-Inclusive Care for the Elderly (PACE). This position is responsible for all physical therapy programming for PACE North participants.  Duties include conducting evaluations and assessments, providing and recommending treatments, care planning, supervision of program aides and other staff implementing treatment plans, assisting with durable medical equipment purchasing, developing and implementing group exercise programs, and membership on the Interdisciplinary Team.

The ideal candidate will be a licensed Physical Therapist in the state of Michigan with current CPR certification, valid MI driver’s license, ability to work effectively in a team environment, and a minimum of 1 year of experience working a frail or elderly population.

Drivers - Full & Part Time

Traverse City, MI

PACE North is seeking Drivers for its new Program of All-Inclusive Care for the Elderly (PACE). Duties include transportation of physically limited passengers to and from their destinations, conducting scheduled inspections of vehicles, and performing routine maintenance as needed.  

The ideal candidate will have a high school diploma or equivalent, valid MI Chauffeur's driving license, and good driving record.  Ability to pass DOT physical, physically operate handicap accessible vehicles and work effectively in a team environment.

Certified Nurse Aides

Traverse City, MI

PACE North is hiring Certified Nurse Aides for its new Program of All-Inclusive Care for the Elderly (PACE).  These flexible positions are responsible for performing personal care activities in participant homes and the Day Center.  Schedule may include some evening and weekend hours.

Candidates must be Certified as a Nurse Aide or willing to participate in paid CNA training.  $15 per hour start wage for certified candidates.  $16 per hour for CNA's with 5+ years of experience.  Join a team of compassionate professionals dedicated to improving the lives of older adults through an innovative, community-based program.
