Education: High school diploma or equivalent preferred.

Licensure/Registry/ Certification: Certified in MI as a CNA. CPR certification. ServSafe Certification. Valid MI driver’s license.  COVID-19 vaccination or approved exemption.  Influenza vaccination or approved exemption.

Experience: Six months clinical experience as a personal care aide, nursing assistant, or emergency medical technician preferred, and a minimum of 1 year experience working with a frail or elderly population preferred.

Skills: Must possess the skills necessary to provide care for geriatric patients. Ability to think and work independently and with direction, and communicate with staff members in a fast-paced and sometimes stressful environment. Ability to function well in a team environment. Possess basic computing skills in the workplace. Act within the authorized scope of practice. Responsibilities:

Provide basic ADL and IADL care to assigned participants, including bathing, dressing, assist with eating, in addition to taking vital signs, repositioning, and answering call lights.

Assist with preparation, serving, and clean-up of meals and snacks.

Laundry tasks as assigned.

Assist with stocking and maintaining personal care supplies for use in the day center.

Assist with restorative therapies and recreational programming.

Participate in the activities of the Interdisciplinary Team as needed and carry out the plan of care as directed.

Attend and participate in scheduled staff meetings and participant care meetings as requested.

Assist with transport of participants inside and outside the day center.

Accompany participants to appointments.

Document treatments/procedures, observations, personal care, and participant activities on appropriate forms and in the medical record as needed. Examples include vital signs, food intake, medication reminders, and participant complaints of physical symptoms.

Operate durable medical equipment safely and effectively according to each participant need.

Report changes in the participant’s condition to the appropriate Interdisciplinary Team member.

RN - Nurse Care Manager

Traverse City, MI

PACE North is seeking a Nurse Care Manager to join our amazing team of professionals dedicated to improving the lives of older adults in our community, through an innovative, center-based program.

This position is responsible to develop, implement, and monitor homecare services for PACE North participants.  Duties include conducting initial and periodic assessments of participants' home care & safety needs, coordinating care delivery, providing in-home nursing care and prescribed treatments, authorization of durable medical equipment purchases, supervisory oversight of Homecare Aides, and membership on the Interdisciplinary Team.

The ideal candidate will be a licensed in the state of Michigan as a RN with a valid MI driver’s license and current CPR certification.  Minimum 2 years recent experience working as a nurse, preferably in home or community health, in addition to 1 year of experience working with a frail or elderly population. Ability to work effectively in a team environment.  COVID-19 vaccination or approved exemption.  Influenza vaccination or approved exemption.

Assists passengers on and off the van in a safe and courteous manner and escorting participants safely to the door. Deals with medical equipment and other emergencies that may arise.

Inspects van and maintains it in a clean, orderly, and safe condition. Reports the need for maintenance.

Reports changes in a participant’s home situation or behavior to Transportation Coordinator.

Provides pick-up and delivery service including meals, medications, laundry, incontinence products, DME, etc.

Demonstrates active participation in QI plan processes.
