  • Program of All-inclusive Care for the Elderly
  • PACE is an innovate model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live an independently as possible.
  • PACE programs coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community.
  • PACE utilizes an interdisciplinary teams – including physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – to exchange information and solve problems as the conditions and needs of each individual who decides to participate in PACE change – all with the objective of enabling participants to live longer in the community.

Ideal candidates will be Certified as a Nurse Aide or willing to participate in paid CNA training. $15 per hour start wage for certified candidates.  $16 per hour for CNA's with 5+ years of experience.  Mileage reimbursement (.575 cents per mile).  Benefits include great work environment + medical, dental, vision coverage, paid time off, and 401K with match for full time staff.

Caregiver - PACE Day Center

Help us help others stay at home!  PACE North is actively recruiting a full time Caregiver to provide direct care to our Participants in the Day Center.  Duties may also include caring for participants in their homes and occasional evening or weekend hours.

RN - Care Management

Traverse City, MI

PACE North is seeking a RN - Care Manager to join our amazing team of professionals dedicated to improving the lives of older adults through an innovative, community-based program.

This position is responsible to develop, implement, and monitor homecare services for PACE North participants.  Duties include conducting initial and periodic assessments of participants' home care & safety needs, coordinating 24-hour care delivery, providing in-home nursing care and prescribed treatments, authorization of durable medical equipment purchases, coordination of therapy services and nursing home care, supervisory oversight of Homecare Aides, and membership on the Interdisciplinary Team.

The ideal candidate will be a Registered Nurse licensed in the state of Michigan with a valid MI driver’s license and current CPR certification.  Minimum of 1 year of experience as an RN, in addition to 1 year of experience working with a frail or elderly population. Ability to work effectively in a team environment.

Clinic Assistant

Traverse City, MI

PACE North is seeking a Clinic Assistant to join our amazing team of professionals dedicated to improving the lives of older adults through an innovative, community-based program.

This position is responsible to perform specified healthcare services and clerical duties in the PACE North primary care clinic.  The ideal candidate will be certified as a nurse aide in Michigan, with phlebotomy certification or training.  Knowledge of medical terminology & scheduling.  Strong clerical, word processing, and customer services skills.  Experience working with a frail or elderly population.  Ability to work effectively in a team environment.

This full time position offers a start wage of $16.50 per hour, plus a comprehensive benefits package including health, dental, and vision coverage, generous paid time off system, 401K retirement plan with employer match, and longevity bonus.
