PACE North is seeking a Community Liason for its new Program of All-Inclusive Care for the Elderly (PACE), which will provide innovative care for aging individuals in the northwest Michigan region. This position is responsible for evaluation, assessment, and coordination of new enrollees into the PACE program.  Duties include acting as first point of contact for inquires about the program from the general public and referral sources including hospitals, home health programs and nursing facilities, in addition to conducting telephone and in-home assessments, scheduling site visits, and coordination of level of care decisions.

The ideal candidate will have a degree or relevant experience in health care or social services related field, Michigan licensure as required, valid MI driver’s license, and the ability to work effectively in a team environment.

RN - Nurse Care Manager +$5,000 sign on bonus

PACE North is seeking a Nurse Care Manager to join our amazing team of professionals dedicated to improving the lives of older adults in our community, through an innovative, center-based program.

This position is responsible to develop, implement, and monitor homecare services for PACE North participants.  Duties include conducting initial and periodic assessments of participants' home care & safety needs, coordinating care delivery, providing in-home nursing care and prescribed treatments, authorization of durable medical equipment purchases, supervisory oversight of Homecare Aides, and membership on the Interdisciplinary Team.

Reports changes in a participant’s home situation or behavior to Transportation Coordinator.

Communicate effectively with participants, families, staff, and the staff of other agencies.

Provide basic ADL and IADL care to assigned participants, including bathing, dressing, assist with eating, in addition to taking vital signs, repositioning, and answering call lights.

Assist with preparation, serving, and clean-up of meals and snacks.

Laundry tasks as assigned.

Assist with stocking and maintaining personal care supplies for use in the day center.

Assist with restorative therapies and recreational programming.

Day Center Supervisor - LPN

Under the direct supervision of the Center Director, you will oversee participant care activities and other tasks as assigned to Certified Nursing Assistants in home care and Day Health Center settings.  

 

  • Responsibilities

  • 1. Hires, supervises and evaluates performance of PACE North CNAs. 

  • 2. Oversees schedules and provides structure to Day Health Center CNAs, ensuring that time is effectively utilized.  

  • 3. Supervises DHC Lead CNA/Home Care Assistant and assists as needed to ensure their tasks are completed. 

  • 4. Directly supervises and oversees scheduling of all PACE North CNAs.  

  • 5. Works collaboratively with CNAs to perform participant care as needed.  

  • 6. Manages CNA responsibilities with Home Care Coordinator to ensure home care services are being provided compliantly by CNA staff. 

  • 7. Completes annual competency checks for all CNAs. 

  • 8. Provides CNA education on relevant topics. 

  • 9. Monitors participants’ health needs, working with triage nurse/clinic staff. 

  • 10. Communicates effectively with participants, families, staff and vendors. 

  • 11. Assists with evaluations and human resource management of Certified Nurse Aides, in conjunction with the Center Director.
