What is PACE?  

  • Program of All-inclusive Care for the Elderly
  • PACE is an innovate model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live an independently as possible.
  • PACE programs coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community.
  • PACE utilizes an interdisciplinary teams – including physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – to exchange information and solve problems as the conditions and needs of each individual who decides to participate in PACE change – all with the objective of enabling participants to live longer in the community.

RN - Care Management

Traverse City, MI

PACE North is seeking a Nurse Care Manager to join our amazing team of professionals dedicated to improving the lives of older adults in our community, through an innovative, center-based program.

This position is responsible to develop, implement, and monitor homecare services for PACE North participants.  Duties include conducting initial and periodic assessments of participants' home care & safety needs, coordinating 24-hour care delivery, providing in-home nursing care and prescribed treatments, authorization of durable medical equipment purchases, supervisory oversight of Homecare Aides, and membership on the Interdisciplinary Team.

The ideal candidate will be a licensed in the state of Michigan as a RN with a valid MI driver’s license and current CPR certification.  Minimum 2 years recent experience working as a nurse, perferably in home or community health, in addition to 1 year of experience working with a frail or elderly population. Ability to work effectively in a team environment.

Drivers - Part time / Flexible

Ideal candidates will have a high school diploma or equivalent, valid MI Chauffeur's driver's license, and good driving record.  Ability to pass DOT physical, operate handicap accessible vehicles, and work effectively in a team environment. $14.35 per hour + great work environment + great benefits including medical, dental, vision coverage, paid time off, and 401K with match.
