An Interdisciplinary Team (IDT) of experts works with each participant to determine a customized care plan. The team includes a Physician; Registered Nurse; Home Care Coordinator; Masters-level Social Worker; Occupational Therapist; Physical Therapist; Recreational Therapist; Registered Dietitian; PACE Center Director; Certified Nurse Assistant; and Transportation coordinator.
This team focuses on "you" and makes decisions regarding primary medical care to recreational therapy to nutrition to medicines to medical equipment to decisions on the types of local home chore services you may need. And even more. When necessary, the plan will include acute care, hospitalization, and nursing care -- all paid for by your PACE healthcare insurance plan.
You will receive most of the medical services at our Day Center where there will also be time for games, hobbies, and laughing with new friends while enjoying delicious hot lunches. When you're not at the Center, services deemed necessary by your Interdisciplinary Team will be provided to you in your home.
