PACE is truly all about you and your specific medical & wellness needs

An Interdisciplinary Team (IDT) of experts works with each participant to determine YOUR customized care plan. The team includes a Physician; Registered Nurse; Home Care Coordinator; Masters-level Social Worker; Occupational Therapist; Physical Therapist; Recreational Therapist; Registered Dietitian; PACE Center Manager; Certified Nurse Assistant; and Transportation coordinator.

This team focuses on "YOU" and makes decisions regarding your "Plan of Care" including, but not limited to:

  • Primary medical care
  • Recreational therapy
  • Nutrition
  • Medicines
  • Medical equipment
  • Home chore services 
  • Acute care (hospitalization and nursing care) 

All PACE services are paid for by your PACE healthcare insurance plan.  There is NO out of pocket expense for any approved care, service, equipment/supplies.

You will receive most of the medical services at our Day Center where there will also be time for games, hobbies, and laughing with new friends while enjoying delicious hot lunches. When you're not at the Center, services deemed necessary by your Interdisciplinary Team will be provided to you in your home.
