How I can Qualify

You can join PACE North if you meet the following conditions:

✔ You are 55 years old or older.

✔ You live in the service area for PACE North.

✔ You are certified by the State of Michigan to need nursing home level of care.

✔ You are able to live safely in the community, with the help of PACE services when you join.

A potential PACE enrollee may be any or all of the following:

• Entitled to Medicare Part A
• Enrolled under Medicare Part B
• Eligible for or enrolled in Medicaid
